ELIGIBILITY CRITERIA

GENERAL

  1. Resident of Punjab
  2. Possess a valid CNIC or B-Form, or valid CNIC of guardian in case of minor or intellectually impaired (A copy of the identification document must be attached with the application).
  3. Disability Assessment Certificate issued by the Disability Assessment Board, in case of wheelchairs, prosthetic, orthotics and other such assistive devise except hearing aids.
  4. Recommendations of Need Assessment Committee for all category of assistive devices.

  • HEARING AIDS

Eligibility Criteria:

  1. Audiometric value between 50-80 dB (moderate-severe impairment as per WHO guidelines).
  2. Must receive a need assessment and recommendation from the Hearing Aids Need Assessment Committee.
  3. ORTHOTIC AND OTHER DEVICES

         Eligibility Criteria:   

  1. Must have Disability Certificate
  2. Must have good condition
  3. Good life expectancy (more than 3 years)
  4. Must have respective fitness
  • TRI MOTORCYCLE

         Eligibility Criteria:

  1. 18-45 years of age
  2. Employed either in public or private sector (must attach evidence)
  3. Present some viable business option/ employment opportunity (evidence required)
  4. Active/mobile PWDs who demonstrate a need for enhanced mobility (need assessment by orthopedic) Inpiduals with severe mobility impairments who cannot use traditional mobility aids (need assessment by orthopedic).
  5. Have intermediate or above

APPLICATION PROCESS

  1. The Department will invite applications for provision of Assistive Devices, by mentioning all types, through an open advertisement in prominent national newspapers, electronic and social media.
  2. The applicants shall submit through online application on the portal developed by the SW&BM department in collaboration with PITB, link of the portal shall also be given in the advertisement.
  3. Applications received through Maryam ki Awaz will also be considered.
  4. Each applicant can select only one assistive device or one required prosthetic from the list given on the portal.
  5. Persons with Disabilities meeting eligibility criteria can submit application for provision of Wheelchairs, Prosthetic, Orthotic and other such Assistive Devices, whereas, anyone meeting the eligibility criteria can submit application for provision of Hearing Aids.

PHYSICAL VERIFICATION OF THE APPLICANT 

  1. Disability Certificate verification
  2. Address verification
  3. Mobile number verification
  4. In persons verification of the applicant including current picture
  5. Applicant need assessment verification as recommended by concerned orthopedic or audiologist.
